Hāpai Te Hauora Tāpui collects the personal information you provide in this form for the purposes of administering the World Smokefree May 2026 campaign, contacting you about your participation, managing prize draws, and providing support or referrals where you request them.

The information collected may include your name, contact details, date of birth, region, gender, ethnicity, smoking or vaping status, pregnancy-related information, support preferences, address details where relevant, and any information you choose to share about your reason for quitting.

If you request support from a stop smoking service, the information needed for that referral may be shared with the service you select so they can contact you and provide support.

Your personal information will only be used for the purposes for which it was collected, for directly related purposes, with your consent, or where required or permitted by law. It will be stored securely and accessed only by authorised staff and service providers supporting this campaign.

Some information is required for us to register your participation and, where applicable, contact you if you are selected as a prize winner. If you do not provide required information, we may not be able to process your entry, include you in prize draws, or contact you about the campaign.

We may also use de-identified or aggregated information for reporting, evaluation, and future campaign planning.
